Why are nursing care plans so important?

They help to assess how the patient is, their pain score and to aid in keeping their routine as normal as possible. It helps to ensure patietns are receiving care which is not only effective but also taking into account their own individual preferences.

How to complete nursing care plans:

I have added a gallery below showing examples of how each care plan needs to be completed. This will vary between patients and they are just examples (patient names and case references have been blanked out for GDPR).
